How do I sign up for the membership?

What is included in my membership?

Memberships will differ based on the level of membership, whether you select the Weight Management, Signature, and Elite membership.

Each membership includes our core services:

  • An initial HMW Method Wellness Assessment
  • Ongoing wellness and nutrition visits with your HMW care team
  • Medication management and supplement recommendations
  • A personalized meal plan
  • Individualized exercise program
  • Wearable data analysis
  • Wellness education and health coaching
  • Access to HMW’s patient care coordinator to help facilitate medical needs and medications refills

Memberships also include access to our HMW member app. where you can view your personalized wellness program, direct message your HMW providers, and access our library of yoga and meditation videos. The Signature and Elite membership’s include primary care services and additional health focuses which will be more comprehensive in scope and care.

What is not included in my membership?

While we will help coordinate many services, some of their associated costs are not covered under your membership but will be billed through insurance. Those services would include laboratory and functional medicine testing, wearable devices, and prescriptions. Additionally, your HMW team may recommend certain nutraceuticals/supplements that are not covered under the membership. You will receive a 10% discount on all supplements through our affiliated supplier. Our HMW memberships do not cover diagnostic images, specialist visits, emergency room visits, or hospitalizations. 

Is the membership a full replacement for primary care?

The Signature and Elite Holistic Health Memberships fully replace primary care services. The Holistic Weight Management membership is structured to support weight management but does not include standard primary care providers' services such as annual physicals, vaccinations, sick visits, or refills on medications prescribed outside of our practice. 

Who is on my care team?

Your HMW care team is composed of a Physician, who is a board-certified Medical Doctor and Institute for Functional Medicine Practitioner, a Registered Dietitian with Functional Medicine training, and our Patient Care Coordinator. 

Do you offer virtual memberships?

Yes, we offer telehealth services in New York, New Jersey, Florida, Wisconsin, and Wyoming.

All members receive an account through our member app, which allows virtual access to a personalized meal plan and exercise program, and a library of yoga and meditation videos that can be done from anywhere.

What should I expect from my first appointment?

Your first appointment includes our baseline HMW METHOD Wellness Visit. This begins with a comprehensive initial wellness session with your HMW Physician. This session will include a discussion of your health and wellness goals, a review of your health history, and a root cause analysis of your current symptoms. In the office, we will perform a physical examination, biometric analysis, and a strength assessment. Then you will meet with your HMW Dietitian for a consultation and nutrition analysis. Lastly, we do onsite phlebotomy for labs. 

What types of examinations or procedures do the assessments include?

Our memberships include physical examinations, biometric analysis, and strength assessments. Our physical examination allows our physician to check for overall health and signs of disease; we monitor vital signs, such as heart rate and blood pressure. Our biometric analysis is done using our Inbody 770 scale which assesses body composition, including muscle mass, body fat percentage, basal metabolic rate, and more.

If you are a virtual member, we may recommend purchasing a biometric scale at home. Our strength assessment includes a series of tests completed either in person or virtually to assess muscular strength, muscular endurance, and aerobic endurance.

How do I access my individualized programs?

Your nutrition and exercise programs are available through our Member App. You will receive access to the member app during your initial assessment.

How will I be billed?

Memberships are billed monthly through credit card payments or ACH. Memberships require a 6-month minimum commitment. 

What happens if I need to cancel my membership?

Once enrolled in a membership, your membership will remain active for 1 year unless canceled. Your membership can be canceled at any time with 30 days' notice after the minimum 6-month commitment is met. If you choose to pay your annual membership in full, you will be refunded for the unused months after the 6-month cancelation window. We will work with you to ensure that you’re satisfied with your membership!
