What is not included in my membership?
While we coordinate many services as part of your membership, certain associated costs are not included and will be billed through your insurance. These may include laboratory and functional medicine testing, wearable devices, and prescriptions. However, some care packages do include expanded testing options. Additionally, your HMW team may recommend nutraceuticals or supplements that are not covered by your membership. As a member, you’ll receive a 10% discount on all supplements purchased through our affiliated supplier. Please note that HMW care packages do not cover diagnostic imaging, specialist visits, emergency room visits, or hospitalizations.
Are the care packages a full replacement for primary care?
The comprehensive nature of our care packages does provide holistic support for patients. However they do not include standard primary care provider services such as annual physicals, vaccinations, sick visits, or refills on medications prescribed outside of our practice.
Who is on my care team?
Your HMW care team is composed of a Physician, who is a board-certified Medical Doctor and Institute for Functional Medicine Practitioner, a Registered Dietitian with Functional Medicine training, and our Patient Care Coordinator.
Do you offer virtual memberships?
Yes, we offer telehealth services in New York, New Jersey, Florida, Wisconsin, and Wyoming. All members receive an account through our member app, which allows virtual access to a personalized meal plan and exercise program, and a library of yoga and meditation videos that can be done from anywhere.
What should I expect from my first appointment?
Your first appointment with most of our care packages includes our baseline HMW METHOD Wellness Visit. This begins with a comprehensive initial wellness session with your HMW Physician. This session will include a discussion of your health and wellness goals, a review of your health history, and a root cause analysis of your current symptoms. In the office, we will perform a physical examination, biometric analysis, and a strength assessment. Then you will meet with your HMW Dietitian for a consultation and nutrition analysis. Lastly, we do onsite phlebotomy for labs.
What types of examinations or procedures do the assessments include?
Our memberships include physical examinations, biometric analysis, and strength assessments. Our physical examination allows our physician to check for overall health and signs of disease; we monitor vital signs, such as heart rate and blood pressure. Our biometric analysis is done using our Inbody 770 scale which assesses body composition, including muscle mass, body fat percentage, basal metabolic rate, and more.
If you are a virtual member, we may recommend purchasing a biometric scale at home.
How do I access my individualized programs?
Your nutrition and exercise programs are available through our Member App. You will receive access to the member app during your initial assessment.
How will I be billed?
Care Packages are billed through credit card payments or ACH.
What happens if I need to cancel my membership?
Once you enroll in a care package, your membership will remain active for the specified duration of the package, either 2 or 6 months.
